How do I check benefits and/or eligibility?

Ohio Health Choice is not a claims payor and does not have the capabilities to provide benefit information and the proper eligibility status. Please contact the payor at the number listed on the member’s ID card to obtain this information. If you are unable to locate the payor’s phone number, please contact the OHC Customer Service Department at 800.554.0027 with the employer group number and we will be able to assist in providing this information.


How do I check claim status?

To check whether Ohio Health Choice has received or processed a claim, please contact the Customer Service Department at 800-554-0027. Ohio Health Choice is not a claims payor. For claim payment status, please contact the payor at the number listed on the member’s ID card. If you are unable to locate the payor’s phone number, please contact the OHC Customer Service Department with the employer group number and we will be able to assist in providing this information.


How do I file a claims appeal?

Call the payor at the number listed on the member’s ID card for questions related to adjudication issues, claim denials, benefits, UM or eligibility. Contact the Ohio Health Choice Customer Service Department at 800-554-0027 for any questions regarding contracted rates, billing guidelines or network status. The claims appeal process is explained in detail in the “Claim Payment Resolution Issues” section of the Provider Manual (available here).


How do I join the network?

Information about joining the Ohio Health Choice network, along with the necessary forms, can be found on the Provider Resources page.


Am I eligible to join the network?

Ohio Health Choice generally only contracts with providers who have MD, DO, DDS (oral surgeons only), DPM, OD, PA, CNP, LISW, PHD or LPCC titles. Other factors considered when deciding which providers should be added to the network include specialty, geographic location, and continuity of care.


How do I check Provider network status?

To check provider network status, please contact the OHC Customer Service Department at 800-554-0027 where a search can be done to provide status to your inquiry and if necessary, connect you with Ohio Health Choice’s Provider Relations Department for additional verification.


How do I update my demographic information?

Any information changes to your business practice (i.e. change of address, updated tax ID, etc.) must be submitted in writing and either faxed or emailed to the Ohio Health Choice Provider Relations Department. To update your demographic information, a form can be found on the Provider Resources page.
